PSEUDOHYPOALDOSTERONISM: A CASE REPORT

چکیده
INTRODUCTIONPseudohypoaldosteronism is a heterogeneous group of disorders manifested by end-organ resistance to mineralocorticoids. We describe case recurrent salt wasting, hyperkalaemia and metabolic acidosis from this condition. RESULTSA 2.5-year-old girl was first presented us at day 8 life with poor feeding, lethargy in circulatory collapse requiring cardiopulmonary resuscitation. She not dysmorphic, had unremarkable systemic findings normal female genitalia. Laboratory parameters showed persistent hyponatraemia, hyperchloraemic acidosis. initially treated steroid therapy, but shown no clinical improvement. The only significant hormonal work-ups were significantly elevated Renin Aldosterone levels. Genetic confirmation test unfortunately available. Other secondary causes adrenal also excluded. subsequently few more episodes salt-losing crisis, improvement terms severity the frequency event. These successfully high dose sodium supplements potassium-binding resin. now growing along low growth percentile curve reasonable agecorresponding developmental milestones. CONCLUSIONSalt wasting crisis carries mortality morbidity children. Despite being rare, condition such as mineralocorticoid disorder must be considered when response treatment does follow common pattern.

A 41-year-old man, complaining of leg cramps, was found to have persistent hyperkalemia. Except for mild hypertension, his physical examination and laboratory values to exclude connective tissue diseases and diabetes mellitus were normal. Renal function testing revealed a normal glomerular filtration rate and tubular capacity to acidify and dilute, as well as near-normal ability to concentrate ...
متن کاملSecondary or Transient Pseudohypoaldosteronism Associated With Urinary Tract Anomaly and Urinary Infection: A Case Report
Hyponatremia with hyperkalemia in infancy is a rare presentation, but may be due to aldosterone deficiency or end organ resistance to its action. There are few cases associating this condition with urinary tract infections or anatomic abnormalities that predispose to infection. Clinicians should have a high index of suspicion in diagnosing secondary pseudohypoaldosteronism (PHA) due to its ofte...

Systemic pseudohypoaldosteronism (PHA) type I is a rare genetic disorder resulting from mutations in the subunits of the epithelial sodium channel that manifests as severe salt wasting, hyperkalemia, and metabolic acidosis in infancy. In this article we report a patient with systemic PHA type I presenting with severe dehydration due to salt wasting at 6 days of life. She was found to have a kno...
