PSEUDOHYPOALDOSTERONISM: A CASE REPORT

INTRODUCTIONPseudohypoaldosteronism is a heterogeneous group of disorders manifested by end-organ resistance to mineralocorticoids. We describe case recurrent salt wasting, hyperkalaemia and metabolic acidosis from this condition. RESULTSA 2.5-year-old girl was first presented us at day 8 life with poor feeding, lethargy in circulatory collapse requiring cardiopulmonary resuscitation. She not dysmorphic, had unremarkable systemic findings normal female genitalia. Laboratory parameters showed persistent hyponatraemia, hyperchloraemic acidosis. initially treated steroid therapy, but shown no clinical improvement. The only significant hormonal work-ups were significantly elevated Renin Aldosterone levels. Genetic confirmation test unfortunately available. Other secondary causes adrenal also excluded. subsequently few more episodes salt-losing crisis, improvement terms severity the frequency event. These successfully high dose sodium supplements potassium-binding resin. now growing along low growth percentile curve reasonable agecorresponding developmental milestones. CONCLUSIONSalt wasting crisis carries mortality morbidity children. Despite being rare, condition such as mineralocorticoid disorder must be considered when response treatment does follow common pattern.
